Annual Health and Medical Record (Parts A & B) Required Required for all participants. Contains health history, consent to treat, and general information. For: Adults, Youth	Adults, Youth	2019
Annual Health and Medical Record (Part C) Required Required to be filled out by a Physician. Needed for events lasting more than 72 hours. For: Adults, Youth	Adults, Youth	2019
Activity Consent Form Required Parental consent for participation in Scouting activities. For: Youth	Youth	2022
Raven Knob Outdoor Release Required Parental consent for participation in activities at Raven Knob. For: Youth	Youth
Photo Release Form Permission to use photos or videos in Troop 400G publications or media. For: Youth	Youth	2024
CRK Special Diet Form Let CRK know about any special diet requirements for summer camp. For: Youth, Adults	Youth, Adults
